In order to use 45385 does it have to be done by snare?

Yes, CPT 45385 must should be done by snare. If it is hot biopsy forceps/bipolar cautery, use 45384. If it is not amenable to removal by hot biopsy forceps, bipolar cautery or snare technique, use 45383.
