NOTES: endoscopic repair of gastrocutaneous fistula Procedure: A previously placed G-J tube was seen. The tube was removed. The gastrostomy tract was then de-epithelialized with APC. The tract was then closed with two over the scope clips. The exam of the stomach was otherwise normal. What code do use. CPT 43247 REMOVAL?

Previously placed G-J tube was removed, which not to be counted as foreign body removal as mentioned in CPT code 43247. CPT code 43247 is for esophagogastroduodenoscopy, flexible, transoral; with removal of foreign body(s). There is no such specific CPT code for the procedure performed, so CPT code 43999 (unlisted procedure, stomach) is appropriate code to bill for the procedure performed. Attach the operative report with the unlisted code to avoid in deduction of payment.
