The office had a patient here for over one hour with our nurse practitioner. She was counseled in regards to her overuse of pain medications and also charged a new patient visit. Her insurance denied the counceling charge but I think I may have used the wrong code I used 99403. Should I have used something else?

I think you should use 99404 which is approx 60 minutes and most importantly, you should bill the ICD9 code using a V code or a diagnosis code for the preventative care you are providing.
