correct use of modifiers -33 and -PT

when screening colonoscopy becomes surg/diag for medicare pts what is the correct way to use the -PT modifier? same senario for non-medicare pts what is correct way to use -33 mod? is screening dx code primary with polyp etc code secondary? i am getting mixed info from other coders on this issue.
