Yes, you should mention LT/RT to specify which eye the ophthalmologist worked on. Also there are four modifiers related to eye such as E1-E4. Based upon optholmologist's work, you could append these modifiers.
E1: Upper left eyelid
E2: Lower left eyelide
E3: Uppper right eyelid
E4: Lower right eyelid
