Consult hospital setting

My provider was informed, if our established patient was admitted to the hospital with a new symptom, one or more that has not been an issue prior to any other claims we can bill this new service as a consult due to new symptom. It was my understanding once patient is established we must bill as follow up visit only. Please clarify

If your established patient was admitted by some other provider and requesting your consultation, than you should bill the Inpatient consultation E/M code irrespective of, whether the patient is new or established to you. And, if you are the admitting provider, you should bill the appropriate Inpatient Hospital E/M code.
