Patient is status post posterior lateral fusion without instrumentation L4-L5 in November 2019. Patient now has worsening spondyololisthesis at L4-L5 so provider brings the patient back for a computer-assisted (Medtronic Mazor X robot) insertion of percutaneous pedicle screws and rods a L4-L5 bilaterally. I've determined the add on code to be 22840 and 61783 but what would be the primary code?

Thanks for the question.

 

Spondylolisthesis is an abnormal forward displacement of a vertebra, one of the interlocking bones of the spine.

 

In the above scenario, the patient is presenting for L4-L5 spondylolisthesis within the global period of posterior lateral spinal fusion.

 

If the provider is performing reduction of displaced vertebral segment, then code 22325 can be used as a primary procedure with modifier 78.

 

Append modifier 78 to an unplanned procedure that requires the patient to return to the operating room. The unplanned procedure is related to an initial procedure. The provider performs the unplanned procedure during the initial procedure’s postoperative period.

 

The add on codes 22840 and 61783 should be added.
