Colon code help needed

Patient had prior surgry 2 days before - wound is open with a VAC dressing. Lots of adhesions already. Now Resection proximal transverse colon & electd to do a colorectal anastomosis of the left colon to the rectal stump but not to hook up to the small bowel , making it a long Hartman's pouch. Then small bowel Terminal Ileum was in discontinuity ischemic & resected. End ileostomy was done from ligmanet of Treitz & a stoma applied. Because the abdomen was already open should I use 44143.78,52 for the long Hartman's pouch? Or 44140.78.52? What do I use for the terminal ileum resection with the end ileostomy and stoma?

We do not have any guidelines as such to use modifier -52 if previous surgery is performed and current surgery is being performed with previous opening. Modifier -78 which you used, is giving enough information regarding the scenario.
