Coding "Screening" exams and final diagnosis question

In all coding classes we have ever attended, it has been told to us that although a patient is referred for symptom or a screening exam, the radiology coding guidelines state that we bill on findings rather than symptom or screening code when a final diagnosis is available. We cannot find these guidelines listed anywhere. Can you tell us where we can locate Outpatient Radiology Billing Guidelines?
