Coding an E/M code (99212) with bilateral cerumen removal (69210) for ear wax.

A pediatrician sees a patient for cerumen removal in both ears.

1.) Pediatrician selects diagnosis code 380.4 (cerumen impaction)
2.) Selects E/M code 99212 and procedure code 69210 (cerumen removal)

This was what I came up with:

69210(modifier 50)----380.4

Is that right?

69210(modifier 50)----380.4

Above coding is correct, just use modifier 25 with E/M code 99212 since it is included in code 69210 as per CCI edit.
