Coding a post-partum hysterectomy

We had a patient deliver her baby vaginally and the same day underwent an abdominal hysterectomy. Would this be coded as 58150? And would I add any modifiers like a 78? the procedures were performed by two different providers but they are in the same group practice.Thank you for your assistance.

If the Abdominal hysterectomy was performed during the same session of vaginal delivery, then both services would be separately billable without using any modifier. If the Abdominal hysterectomy scheduled in different session for complication occurred from the vaginal delivery, then the services would be billable with the Modifier 78.

CPT codes 58150-58210 are used for abdominal hysterectomy. Kindly co-relate with your detailed operative report for most appropriate code from 58150-58210 for abdominal hysterectomy performed.

The hysterectomy was done later in the day after the delivery. It was not done at the same time as delivery. Modifier 78 would apply here, correct?

Yes, you can append Modifier 78 with the procedure code performed for the complication that require this procedure to be performed on the same day of delivery.

Some payers do not reimburse procedures performed with global obstetrics package codes. Kindly contact with these payers to determine how they want these services reported, to avoid claim denials and ensure appropriate reimbursement for the services provided.
