CMS Transmittal on anesthesia

I need assistance locating the latest CMS Transmittal regarding what CMS came out with a transmittal in 2005 stating: For the purposes of billing for services furnished in the hospital outpatient dieparment, anesthesia is defined to include:
Loca, regional block(s), Moderate sedation/analgesa (conscious sedation, Deep sedation/analgesia, and General anesthesia.
Is there an update to the 2005 transmittal? Thanks Linda
