CCI SAYS NEVER BILL 69424 W/69421 OR 69440 BUT...

What if the 69424 is on the right ear
and the 69421 / 69440 are performed on the left ear

Then it is okay isn't it ?

Two different ears ??

do I use a modifier with the 69421/69440 or is it not necessary

You can report the CPT code 69421, 69440 and 69424 together with an LT/RT modifier, because work was also performed on Right ear. CCI edit bundles this codes because it checks the condition for a single ear only not for both ear.
