CASTING AFTER SPLINTING

I have a physician who did a surgery procedure and then did a splint after the surgery on the same day. I know the application of the splint with the surgery on the same day is included in the surgery but what if the patient comes back during the global and the physician does casting. Can we bill for the application of the cast? Thank You for your help. Tara

Thanks for your question.  The physician can report the code for the application of the cast and supplies.  The initial cast, strap, or splint is included in the global fee for the procedure. CPT guidelines allow any follow-up care that is related to the same condition and devices used if the definitive treatment has already been performed.  This includes re-casting, splinting, or strapping during the post-operative period.  Medical necessity must be met in order to be reimbursed separately.
