How would I code the following (other than the 64721):

DIAGNOSIS:
1.Right carpal tunnel syndrome.
2.Right lateral epicondylitis.
3.Right radial tunnel syndrome.

PROCEDURE:
1.Right carpal tunnel release.
2.Right radial tunnel decompression (neurolysis, posterior interosseous nerve, proximal forearm).
3.Right lateral epicondyle release with partial lateral epicondyle ostectomy.
