Kathy Posted Wed 25th of November, 2015 18:23:42 PM

Is it appropriate to bill for removal of laparoscopic gastrostomy tube, 43653, in clinic, during the global period for placement of the G-tube?

Is it appropriate to bill for cautery of granulated flesh at site of G-tube, during global period for placement of G-tube?

Please site Coding Compliance Guidelines and resources. Thank you.

Thanks for your query,

If the surgeon who originally billed the laparoscopic gastrostomy tube placement, does removal of laparoscopic gastrostomy anywhere, it cannot be billed. It is included in the E/M. No need to bill this with unlisted code or any other code.

Thank you. Would you please clarify 2 things:

1. You indicated the removal of the gastrostomy tube is included in the E/M... Are you saying, because the Provider recognized that he needed to cauterize the granulated tissue, the E/M is billable with a -24?
2. You didn't answer my question about the cautery of granulated flesh at the G-tube site. Would this be billable with a -79?

Thank you.

1. As per my understanding, we will not use modifier -24 with E/M service because removal of G-tube is not an unreleated procedure.

2. Cautery of granulated flesh at the G-tube site would be billable with 79 modifier.
