Hi Prahlad,

 

These types of cardiac procedures cannot be coded without operative reports. Operative report is must for accurate coding. These procedures involve coding for ICD device with coding for leads along with testing. Per the limited documentation, you should refer to CPT code 33263 (dual leads) and 33264 (multiple leads) for the performed procedure.
