hello, how would this be coded bilateral capsulorrhaphy was peformed with an implant exchange with a smaller implant, pt. had previous breast augmentation, capsule was thin no contracture was noted,

The correct code to bill for bilateral capsulorrhaphy (breast) with implant exchange is CPT 19380 (Revision of reconstructed breast). Also, use modifier 50 to indicate a procedure is performed identically on the opposite side of the body. Feel free to ask for any further query.
