    'Can a routine preventive exam be coded with a tobacco cessation counseling code? Encounter for routine pe, greater than 3 minutes discussing smiking cessation and benifits verses risk. 99395, and 90406?'

    CPT 90406 is an invalid code. If you are talking about 99406, then it should not be billed with preventative exam codes. There are not any specific documentation guidelines in terms of time spent for preventative medicine CPT codes (99381-99397)and it is difficult to find out the time for the smoking cessation. It should not be billed separately when patients present for routine care. You may bill the smoking cessation when billing E&M office visits 99201-99205, 99212-99215, as long as the documentation is separately identifiable and supports both. Use a -25 modifier on the office visit.
