CABG Medicaid Modifiers

Physician performs a CABG x 2 involving a LIMA to the LAD and reverse segment of saphenous vein graft to the obtuse marginal; endo vein harvest technique of the greater saphenous vein right lower side. Codes selected: 33533, 33517, 33508. Medicaid denying due to modifiers missing on 33517 and 33508. Do you have any guidance on modifiers needed for Medicaid on 33517 and 33508?

CPT codes 33517 and 33508 do not require any modifier to get reimbursement from Medicaid. There are not bundling issues between these codes as well. Could you please share your EOB (Explanation of benefit) document received from Medicaid. This will help us identify the reason of denial.
