Which code 11100 or 40490?

Plan: Bx by Shave Method Left lower cutaneous lip. The area was prepped with alcohol. Approx 0.5 cc of 1% lidocaine with epi was infiltrated. A bx by shave method (sent for H&E) was performed using a 15 blade on the left lower lip. TCA 35 % was used for hemostasis. Following the Bx lesion was treated with liquid nitrogen.
