bronchoscopy

Janice Posted Fri 05th of October, 2012 14:45:06 PM

would this be a 31622 ??

the bronchoscope was passed through the existing LMA tube. The vocal cords were visualized. There was some blood spillage noted over the vocal cords. Lidocaine was squirted over the vocal cords. Bronchoscope was then passed into the trachea. Approximately 3 to 4 cm below the level of the vocal cords on the right lateral aspect, there was a mass noted. There were prominent blood vessels over the mass. It is partially occluding the trachea. There was some blood noted over the growth. Bronchoscope was then passed into the trachea below the level of the growth. The main carina was sharp. There was blood spillage noted in the right and left bronchial tree. Bronchoscope was then passed into the right main stem bronchus, into the right upper lobe, right middle lobe and right lower lobe. There was some blood spillage noted, which was suctioned. No active bleeding noticed. No endobronchial masses noted. Next, the bronchoscope was then withdrawn to the main carina, passed into the left main stem bronchus, into the left upper lobe, left lower lobe. There was some blood spillage noted in the left lower lobe, which was suctioned. No active bleeding noticed. Bronchoscope was then withdrawn to the trachea. The mass that was noticed in the subglottic area was again visualized. It would ooze easily. Epinephrine 1:10,000, a total of 25 mL, was squirted over the period of time. Washings were obtained from the trachea. The washings were sent for Gram stain cultures, AB smear, fungal cultures and for cytology. No brushing, no biopsies were obtained. The mass would ooze easily. Since it is in the subglottic area and it is partially obstructing the trachea,
