Coding for botulinum toxin injection would be reported with the 6461X codes depending upon anatomically where the injections were performed, i.e. neck muscles - 64616 or trunk muscles 64646 / 64647 or extremity muscles 64642-64645 The 20550 and 64413 codes are not correct for injections of botulinum toxin. The J0585 code does not require modifier 59 appended and it should not be used indiscriminately. In addition the second and third line items of J0585 needs to be different from 99 units or else the payer will deny as duplicate line items. If necessary to bill with no more than 2 digits in box 24G, the billing for 300 units would be:
J0585 with 99 units
J0585 with 98 units
J0585 with 97 units
J0585 with 6 units
