Bone marrow pelvic aspirate with grafting to Left femur secondary to non union

Help!! I need to code for a Bone marrow pelvic aspirate with grafting to Left femur secondary to a fracture non union. This would be autologous bone marrow aspirate taken at time of grafting into femur. I see 38220 is for bone marrow aspirate and I see 38241 for transplant, however, this seems to indicate the harvested material being given as an IV infusion. The doctor is going to insert/transplant this directly into the femur. There is no indication that he is going to do a repair (27472). Any guidance would be appreciated.

 Hi Margaret,

The correct code, when the aspirate is harvested from a separate surgical site via a separate incision for the intent of a fusion is CPT code 38220, Bone marrow; aspiration only. CPT 27472 will be an incorrect code to bill. However, to help you with code 38241 we would like to have more details of procedure note along with diagnosis codes. Thank you
