A: Right foot ulcer excision
-- Ulcerated acral skin with soft tissue inflammation and necrosis
-- Focal bone present, negative for acute osteomyelitis
B: Bone, right foot metatarsal, excision:
-- Portion of bone with reactive changes
-- No definite acute osteomyelitis identified
C: Bone, right foot phalanx, excision:

What level would you assign to the above specimens ? Thank you

Greetings from SuperCoder.com!
If bone fragment from foot was submitted separately from the soft tissue specimen (e.g. Ulcerated acral skin with soft tissue inflammation and necrosis) with a request from the physician to rule out bone disease e.g. osteomyelitis, you may report bone biopsy code 88307.
But if the bone fragment was submitted without a special request report code 88304.
