According to a question and answer in CPT Assistant (January 1999, page 10, Coding Consultation Medicine Pulmonary), you should use the CPAP code (94660). See below for the Q&A:

Question: What is the appropriate code for reporting BiPAP?

AMA Comment: BiPAP is noninvasive mechanical ventilation. It includes continuous positive airway pressure (CPAP) and pressure support ventilation. Since it includes CPAP, CPT code 94660 may be used for the initiation and subsequent management of BiPAP.

EZPAP may be utilized for lung expansion therapy. The correct CPT code would be 94640 if the medical indication of the CPT code is met for treatment of the “acute airway” or “diagnostic sputum induction.”

94640 Pressurized or nonpressurized inhalation treatment for acute airway obstruction or for sputum induction for diagnostic purposes (eg, with an aerosol generator, nebulizer, metered dose inhaler or intermittent positive pressure breathing (IPPB) device)
