Generally, when submitting claims to insurance it may not be necessary to submit a requisition form to the payer. It depends on the payer’s guidelines whether they require the requisition form with claim billing or not. The requisition form does contain quite a bit of information that needs to submit your claim such as the name, dob, patient registration information, billing information, specimen info, and the specimen #.
