Our podiatrist feels that a total contact cast application (29445) can be billed separately from a debridement (97597) when treating the same ulceration. I show they are bundled however, I am reading conflicting articles that state that they can be billed together and that Medicare will pay both codes? Thank you for your help.

Hi Ingrid,

In this case, we can bill 29445 and 97597 together.  Append mod-59 to 29445. Thank you.

Can you tell me why they are allowed to be unbundled?

As per NCCI edits total contact cast application (29445) cannot be billed separately from a debridement (97597) when treating the same ulceration/same anatomical location for medicare as of above scenario. However, Commercial payers may consider separate payment with appropriate modifier. You can send us details of you report to guide you more. Fell free to ask for any further query.
