billing multiple 11300 codes on same visit by same provider on same day

hi there. we have billed 11300 on a patient which we saw on the same day by same doctor. the lesions were all located on the back and 0.5cm. there were 4 lesions. we are getting repeated denials by mey,dicare for duplicate submission.

Originally was billed like this: 11300x4 units and denied for too many units on same patient in 1 day.

claim rebilled after denial and sent out like: 11300x1, 11300x1, 11300x1, 11300x1 with 59 modifier on last 3. this got denied for code 18, exact duplicate claim/service.

claim then rebilled with 11300x1, 11300x1, 11300x1, 11300x1 with last 3 having xs modifier and got denied again.

Bill in this way. 11300x1, 11300-51x1, 11300-51x1, 11300-51x1.
