Billing CPT Codes 11000, 17000 and 17003 together

I am having trouble billing the following codes 11000, 17000 and 17003 together to Medicare. We are trying to bill 17000 (dx 702.0) and 17003 (dx 782.0) and last 11100-59 (dx 238.2). Please advise if this is correct? Thank you.

Hi, As for the scenario mentioned, CPT 17000 (dx 702.0) and 17003 (dx 782.0) and last 11100-59 (dx 238.2) seems to be correct. According to CCI edits, there is a conflict between the CPT codes 17000 and 11000. Hence , we have to append modifier to 11100 to override this relationship. Thus, modifier 59 seems to be appropriate for this scenario. Please check your documentation once, before billing. Hope this Helps. Thanks.
