Billing 44160 with 44180-XS

DX: K635, D12.0 Procedure: Laparoscopic hand assisted mobilization of the colon with conversion to open for anastomosis. Specimens: 1) Distal right colon and terminal ileum, 2) Remainder of right colon end anastomosis. Could I bill 44160 with 44180-XS if laparoscopic was for small bowel and open was for large bowel? Separate incision for proximal right hemicolectomy?

Hi, thanks for your question. As per documentation, this is a single procedure for which you have reported code 44160 (Colectomy, partial, with removal of terminal ileum with ileocolostomy). Your physician first tried to do it laparoscopically, but converted it to open to complete the procedure. As per guidelines, if a procedure is converted from laparoscopic to open, then the open procedure should only be reported. Second reason not to bill both codes together is that there is CCI edit between these two codes and modifier is not allowed. Please feel free to write to us if you have any query or concern. Thanks.
