The question is regarding removal and placement of stents. If a stent is removed from the biliary tree. Then TWO stents are placed into the common bile duct, would you code 43274 and 43274-59 since per cpt guidelines if stents are placed side by side you can report it for both. Or would you code that as 43276 removal and exchange of stents (although ONE was removed and TWO were placed)?
Any help will be appreciated. Thank you!

43276 includes removal and replacement [exchange] of one stent. For replacement [exchange] of additional stent[s] during the same session, report 43276 with modifier 59 for each additional replacement [exchange].

43276 is removal and exchange of stents (each stent exchanged). So even though only ONE was removed and TWO were placed you would code 43276, and 43276-59?
The confusion comes from the Parenthesis statement under cpt 43276:
43276 includes removal and replacement [exchange] of ONE stent. for replacement [exchange] of additional stents during the same session, report 43276 with mod 59 for each additional replacement [exchange].
Since we are exchanging one stent for two. Thanks in advance

Although we are removing one and replacing same duct with Two stents as per above documentation, it is correct to bill 43276 and again bill the same code 43276 with modifiers 52 and 59. Hope it helps.
