How would you code a Bilateral T12, L1, and L2 Transforaminal Epidural Steroid Injection?

I know if you inject T12-L1 you use 64479... I would think you use 64483 for the L1 and 64484 for L2 and you would not need to use 64480 due to the fact that no additional levels were done at the cervical or thoracic levels...

Would it be 64479-50 T12, 64483-50 L1 and 64484-50 L2?
