How would this procedure be coded; would you also code for the endoscopic septostomy and the EVD placement; if so, give all code(s) for this procedure?

You can use CPT code 62161. Burr hole creation will get included in the neurosurgical procedure since introduction of the endoscope was not possible without creation of Burr hole. It is a part of the procedure.

Since this procedure is done for hydrocephalus and not for cyst; wouldn't it be coded with cpt codes 61210 and 62160?

I think 62161 is the more appropriate and specific code. This code is more specific to the approach that was used (endoscopic septostomy) and placement of EVD and removal of excessive CSF.
