do we have to bill this code with an office visit code/E&M code?
if it is done in global period of a surgery but unrelated which mod if any?

No for 92071 you do not have to use an E/M.

A note accompanying new code 92072 advises coders to report an appropriate E/M or General Ophthalmological Services (92002-92014) code for subsequent fittings of keratoconic lenses.

Modifier 79 to be used for any unrelated surgery during Post-OP Period within global period.

I think you misunderstood the 2nd part of the question. which mod is attached to 92071 done in global period to unrelated surgery. mod 79 is for major procedure in po of another surgery. so I thought may be mod 24 but that is for e/m. so do I use any mod at all? again this service was done in po period.

Since you are not using any E/M code with 92071, do not use any modifier.
