PATIENT HAD A DUAL CHAMBER PACEMAKER INSERTION 01/12/19 ON 1/14/19 PACEMAKER LEAD DISLODGEMENT PREVIOUS POCKET WAS PREPPED AND DRAPED . IT WAS ANESTHETIZED WITH LIDOCIANE INCISION WAS REOPENED OLD BLOOD AND SECRETIONS WER SUCTIONED OUT. ARTIAL LEAD WAS DISCONNECTED. USING A J TYPE STYLETTE UNDER GUIDANCE AND FLUOROSCOPY IT WAS REPOSITIONED IN THE RIGHT ATRIAL APPENDAGE AND SCREWED IN PLACE. I DONT KNOW IF THIS IS CORRECT 33216-78, 33234-51. PLEAS ADVISE. As per the documentation provided code 33216 with modifier 78 is appropriate to use. Code 33234 shouldn't be billed because removal of transvenous pacemaker electrode(s); single lead system, atrial or ventricular has not performed instead, only repositioning is done.
