Anterior skull base tumor

Linh Posted Wed 06th of January, 2016 19:04:32 PM

How would you report the following procedures? Only one surgeon was involved. Would you use one code 61512 (for meningioma) or the skull base codes 61586, 69990, and 61601? Path report did confirm meningioma.

Bifrontal craniotomy for resection of meningioma from anterior skull base. Approach was through the sinuses: frontal and ethmoid. Both frontal/ethmoid sinuses were exenterated. The dural was opened and microscope was used to debulk more tumor from the skull base. After tumor removal, the ethmoid sinus was packed with harvested temporalis muscle/fascia and dural seal was applied. Bone graft from cranium used to seal the skull base. Dural was closed w/ neuralon. Bone flap replaced with plates/screws and temporalis fascia reapproximated.

CPT code 61512(Craniectomy, trephination, bone flap craniotomy; for excision of meningioma, supratentorial) is a more appropriate and comprehensive code for the procedure performed.
