How would you code an anterior orbiozygomatic skull base approach; Left frontotemporal craniotomy and removal of foreign body, hematoma; with autologous pericranial duraplasty and repair of skull body?

Use CPT 61313 for evacuation of hematoma and CPT 61570 for excision of foreign body. If the procedures have been performed together than use modifier 52 with the secondary procedure.

Well, No, I would not go for skull base codes, as mentioned codes amalgamate the approach as well.
