Anterior Cervical disc & instrumentation using Zero-P

Mari Posted Wed 06th of March, 2013 20:49:44 PM

I am trying to code C4-5 anterior cervical discectomy, removal previous anterior fixation hardware, and C4-5 Zero-P instrumentation and stabilization. Do I use "63075","22845","22855" & "22851" or "22551","22845","22855" & "22851" or some other combination?

The correct code selections are 63075 (anterior cervical discectomy); 22845 (anterior instrumentation); 22855 (removal of previous anterior fixation hardware); and 22851 (cage or other stabilization device insertion). I believe 63075 is correct instead of 22551 because 22551 represents arthrodesis, and your notes state discectomy. Code 22551 does include discectomy, but arthrodesis is the process of fusing bones that form a joint so that you essentially eliminate the joint. If the surgeon didn't fuse the bones/joint together, I don't think 22551 applies.

Also note that 22845 is a Column 2 code of 22855, according to CCI edits. You can report both procedures during the same session, but append a modifier (such as 59) to 22855 to distinguish the procedures from each other on your claim.
