CPT offers '99381'-'99397' for preventive exams. But Medicare has other codes. For example, it has 'G0402' (Initial preventive physical examination; face to face visit, services limited to new beneficiary during the first 12 months of Medicare enrollment) and added 'G0438' (Annual wellness visit; includes a personalized prevention plan of service (PPPS), first visit) and 'G0439' (Annual wellness visit; includes a personalized prevention plan of service (PPPS), subsequent visit). So the patient's age and payer will be factors in code choice.
