Since the veins were ligated and removed, do I still code only 37761 and 37785 or unlisted? EXCERPT: Approximately 2.5 cm longitudinal incision was made over the previously marked perforator vein region. Electrocautery was used to transect the subcutaneous tissues. A surface varicose vein was immediately identified. This was dissected free with scissors as well as electrocautery for a distance of approximately 2 inches 1 cm below the skin incision distally as well as several large branches that were ligated approximately 1-2 cm from the takeoff of this main vein. It was approximately 4 mm in diameter. The vein was then tied distally and clipped just after this had been divided. We continued our dissection down until this vein and communicated with a more superior vein that was also dissected free, ligated and clipped and divided, and the the center vein communicated with a large perforator vein that was approximately 4.5 mm in diameter with the patient in the supine position. The fascia was cleared on all sides of the vein as it perforated to the fascia. It was then ligated with silk suture and then divided and passed off.

As per provided documentation, CPT codes 37761 and 37785 are appropriate to report in this scenario. You should not report unlisted when there is a code for the procedure.

Thank you for your response. I only suggested unlisted because 37761 (ligation) didn't include removal of the varicose veins or the perforator vein and everything that was done seem to be under the one incision. Anyway, thanks for responding. I will go with the two codes as there isn't any edits. I just wanted to be sure.
