What would the anesthesia code be if MD administered TIVA for left L3-4, L4-5, L5-6 TF ESI?
What about for a Right L2-3, L3-4 and L4-5 MBB?

CPT code for TIVA for left L3-4, L4-5, L5-6 TF ESI is 62311.
CPT code for a Right L2-3, L3-4 and L4-5 MBB is 64493, 64494, 64495.

I need clarification, please. I am getting conflicting information on the following:
If CRNA is providing gen anesthesia for a ESI CESI what ANESTHESIA code would be billed?
Anesthesia Code ESI Caudal?
Anesthesia Code TF ESI?
Anesthesia Code -Facet - MBB?

There are two scenarios with which you can bill the services rendered.
If CRNA provided general anaesthesia for a ESI CESI, appropriate CPT code would be 00630 (Anaesthesia for procedures in lumbar region; not otherwise specified) along with modifier QX (Medical direction of two, three, or four concurrent anaesthesia procedures involving qualified individuals) OR, modifier QZ (CRNA service: without medical direction by a physician).
For nerve block injection based upon the positioning of the patient:
01991(other than the prone position) OR, 01992 (prone position)

Facet - MBB - Anesthesia Code
Also, you mentioned for the CESI the code would be 00630 but wouldn't that be a 00600 (c being cervical) ?

There are no specific Anaesthesia codes available for TF ESI (Trans foraminal Epidural Steroid Injection, and Facet- MBB, hence, you can report the service with CPT code 00630 with appropriate modifier. Also, as per the documentation; the mentioned levels are of lumbar region, therefore, CPT code 00600 would not be the appropriate code for lumbar region, as its specifically for the cervical region.
If CESI stands for (Cervical Epidural Steroid Injection), then basis the medical records, you can report 00600, for cervical region.
For nerve block injection based upon the positioning of the patient: 01991(other than the prone position) OR, 01992 (prone position).
