Code 23680 is used to align shoulder dislocation and repair of humerus neck fracture, which is not the case here. As per provided limited documentation, and in the absence of diagnosis, we can suggest you code 23470 (Arthroplasty, glenohumeral joint; hemiarthroplasty) in which the provider replaces the damaged head of a humerus with a prosthetic implant while the glenoid cavity is left intact.

The diagnosis is Posttraumatic posterior instability of the left shoulder associated with a large engaging reverse Hill-Sachs humeral head defect. The physician also indicates in the op note defect is caused from depressed fracture fragment. Allograft was used in the defect area.
