If a pt was seen only for x-ray, do i use a nurse o/v code, 99212 with the x-ray code or just the x-ray code. please help. thank you.

If the patient is there only for an x-ray and is not being seen by provider or a nurse for another issue you should only use the x-ray.

In order to bill for an E/M service you must meet the documentation requirements.

Hope this helps.
