Patient had a Right C2-3 intra-articular amniotic fluid allograft injection. Would the proper coding be 64999, 77003 and what code should be used for the allograft?

Please provide some more details and technique of procedure for appropriate answer.

Procedure note: "Using intermittent fluoroscopic guidance, and after anesthetizing the skin with 1% buffered lidocaine, a 23-gauge, 3-1/2-inch Quincke tip spinal needle was inserted into the posterolateral aspect of the C2-3 facet joint without difficulty or complication. Appropriate positioning was confirmed, both on AP and oblique views as well as with the injection of 0.2 mL of myelogram-safe contrast dye showing clear right C2-3 arthrogram. Following this, 0.5 mL of amniotic fluid allograft, prepared as per the manufacturer's recommendation without any modification or dilution, was injected into the joint without difficulty or complication. Appropriate positioning within the joint and joint capsule was confirmed by visualized dilution and washout of the previously injected contrast dye. " Is 64490 better for this procedure and bill the Allograph as an unlisted J code. No billing for the Arthrogram since this was only done for localization/guidance.

The appropriate codes for above mentioned procedure would be 64490 and Q4139 (intra-articular amniotic fluid allograft ).

NOTE: Fluoroscopic guidance code is inclusive in CPT 64490 and cannot be coded separately.
