After '67904' levator suture slipped in recovery. Had to go back to OR.

What codes and modifiers are best to code. Dr did bilateral '15823' and bilateral '67904'. In recovery levator suture on left eye slipped. Dr had to bring patient back into OR and reopen wound and replace levator suture.
