Physician documents acute on chronic renal failure. Would 518.84 or 584.9 and 585.9 be the correct diagnosis for this diagnosis?

518.84 would be for respiratory, but for renal, consider this ICD-9 Official Guideline:

Section I.B.10:
Acute and Chronic Conditions
If the same condition is described as both acute (subacute) and chronic, and separate subentries exist in the Alphabetic Index at the same indentation level, code both and sequence the acute (subacute) code first.
