Surgeon performed arthroscopic revision of a previous ACL Repair (29888), and anterolateral ligament reconstruction w/hamstring auto graft from the same side (27427). The two codes are bundled per CCI, but 29888 is allowed to be unbundled w/modifier.
Does this scenario meet the definition of separate structure (XS) or
Unusual non-overlapping service (XU)?

Yes this scenario meets the definition of unusual non-overlapping service (XU).
Modifier XU is allowed with CPT code 29888 as arthroscopic revision of a previous ACL Repair was performed, which is different from anterolateral ligament reconstruction.
Modifier XU represents The use of a service that is distinct because it does not overlap usual components of the main service.
