Dx I20.0
The patient has documented CAD with previous multivessel angioplasty and drug eluting stents, last procedure (2009). He has recurrence of atypical chest pain and equivocal stress echo.
He underwent (2015/11) left heart catheterization, selective coronary angiography and left ventriculography.
Decision was made during the procedure to proceed with two vessel coronary angioplasty and drug eluting stents in the LAD and 1 stent in the Cx. Left radial vascular access.
Is this sequence of CPT codes appropriate?
93458-XU, 92928-LD, 92928-LC

Yes, the add-on code has been bundled with the main code on the MPFS.
