I have had multiple claims for 93454 denied as CO-109 just wanted to make sure I am billing this right
DX 410.71
Insurance, place of service and all else is correct other claims for this patient are being paid please advise if this is billed wrong

As per general guidelines, pattern of the coding is correct, but as it is denied for reason Code CO-109. it means claim/service not covered by this payer/contractor. You must send the claim/service to the correct payer/contractor, so contact your contractor to avoid denial.
